Description
ACTION AND MECHANISM
- [LAXATIVE]. An osmotic laxative that increases the osmotic pressure inside the small intestine, causing the release of water into the intestinal lumen, inducing a rapid bowel movement, generally within four hours. - [ANTACID]. Sodium bicarbonate neutralizes excess gastric acid.
SENIORS
No specific problems have been described, but in these patients, kidney or heart failure is more common.
CONTRAINDICATIONS
- Hypersensitivity to any component of the medication.- [DIARRHEA].- [INTESTINAL OBSTRUCTION].- [APPENDICITIS] or [INTESTINAL PERFORATION].
PREGNANCY
Its efficacy and safety have not been evaluated. However, at the recommended doses, its use during pregnancy is acceptable.
INDICATIONS

INTERACTIONS
- Antiarrhythmics (flecainide, quinidine): studies have shown a possible increase in their toxicity due to decreased excretion caused by alkalinization of the urine. - Non-steroidal anti-inflammatory drugs (flufenamic or mefenamic acid, indomethacin): studies with some antacids (algeldrate, magnesium oxide, calcium carbonate) have shown a decrease in their absorption. - Beta-blockers (propranolol, atenolol): Studies with some antacids (algeldrate, magnesium hydroxide) have shown decreased absorption due to reduced beta-blocker dissolution; therefore, it is recommended to space out their administration. - Digitalis glycosides (digoxin, digitoxin): Studies with some antacids (algeldrate, magnesium hydroxide) have shown decreased digitalis absorption; therefore, it is recommended to space out their administration by 2 or 3 hours. - Ephedrine, Pseudoephedrine: Studies have shown possible toxicity due to decreased excretion from urine alkalinization. - Erythromycin: Some studies have shown increased activity from urine alkalinization. - Ketoconazole: Studies have shown possible decreased absorption. - Methadone: Studies have shown a decrease in Excretion due to urine alkalinization: Methotrexate: Studies have shown an increase in methotrexate excretion due to urine alkalinization. Naproxen: Studies have shown a possible increase in absorption. Quinolones (ciprofloxacin, norfloxacin): Studies with some antacids (algeldrate, almagate) have shown a decrease in their absorption due to the formation of insoluble, non-absorbable complexes in the intestine; therefore, it is recommended to separate their administration by 2 or 3 hours. Rifampicin: Some studies have shown a possible decrease in its serum levels due to changes in its solubility caused by variations in gastric pH. - Iron salts (ferrous citrate, fumarate, ferrous sulfate): Studies with some antacids (aluminum hydroxide, magnesium trisilicate) have shown decreased absorption due to the formation of poorly soluble complexes. Therefore, it is recommended to separate administration by 2 or 3 hours. - Lithium salts (lithium carbonate): Studies have shown a possible increase in renal excretion with a loss of antimanic activity, because some of the sodium may be reabsorbed instead of the lithium. - Salicylates (acetylsalicylic acid): Studies have shown decreased salicylate levels due to increased excretion caused by alkalinization of the urine. - Tetracyclines (tetracycline): Studies have shown the possible formation of non-absorbable complexes in the gastrointestinal tract, thus decreasing absorption. Therefore, it is recommended to separate administration by 2 or 3 hours.
LACTATION
With the use of antacids in general, small amounts of ions may be excreted in breast milk, which do not appear to be sufficient to cause adverse effects in the newborn. Use is accepted, although chronic and/or excessive use should be avoided.
CHILDREN
Its safety and efficacy have not been evaluated, so its use is not recommended in children under six years of age without a prior and accurate diagnosis, since children of that age are unable to describe the symptoms accurately.
GUIDELINES FOR PROPER ADMINISTRATION
The preparation should be dissolved in half a glass of water. As an antacid, it is recommended to take it 20 minutes to 1 hour after meals or when discomfort occurs.
POSOLOGY
- Adults, oral:
* Antacid: 1/2-1 dessert teaspoon/8 hours administered 20 to 1 hour after meals or when discomfort occurs.
In cases of resistant constipation, administer 2 dessert spoons on an empty stomach and repeat the same dose after one hour.
* Laxative: 1-2 dessert spoons at bedtime and the same dose the next day on an empty stomach, allowing, if possible, a few minutes of rest.
- Children over 6 years old: generally, half the adult dose.
PRECAUTIONS
- [RENAL FAILURE]: In cases of renal failure, sodium accumulation can occur, leading to hypernatremia and increased fluid retention. - [HEART FAILURE]: Sodium can cause hypernatremia, leading to alterations in the Na+-K+ ATPase pump, decreasing the effectiveness of cardiac contraction. In addition, water retention can lead to hypervolemia, which increases preload and further impairs heart function. - [HIGH BLOOD PRESSURE]: The sodium content of this medication may increase fluid retention and blood pressure; therefore, its use is not recommended. - [METABOLIC ALKALOSIS], [RESPIRATORY ALKALOSIS]: Sodium bicarbonate is a systemic antacid that is absorbed in the gastrointestinal tract; therefore, its use for a prolonged period may result in metabolic alkalosis. - [COLOSTOMY], [ELECTROLYTE IMBALANCE]: In sodium-restricted diets, it should be noted that each dose contains 682.4 mg of sodium.
ADVERSE REACTIONS
The adverse reactions to this medication are mild and infrequent: - Digestive. Occasionally, flatulence and aerophagia. Rarely, vomiting, diarrhea, and nausea. - Metabolic. Very rarely, after a chronic overdose. At high doses, it can cause metabolic alkalosis (more frequent in patients with renal insufficiency).
