Description
Action and mechanism
Association of local anesthetic (benzocaine) with sympathomimetic
(ephedrine) with vasoconstrictor action.
Indications
[HEMORRHOIDS]
Symptomatic treatment of pain, itching or burning associated with hemorrhoids.
Posology
- Adults, topical rectal: Generally, 3-4 applications daily (morning, evening, and after each bowel movement). If anorectal symptoms (pain, itching, and burning) do not improve within 7 days or bleeding occurs, the clinical situation should be reassessed.
- Elderly: Reduce the dose in patients over 65 years of age and debilitated patients, due to an increased risk of systemic toxicity.
Rules for correct administration
Topical rectal application. Carefully clean the affected area with mild soap, dry without rubbing, and apply the ointment using a clean gauze pad.
For intrarectal application, screw the cannula onto the tube. Before inserting it, lubricate the anal sphincter with the same ointment to facilitate penetration.
Contraindications
[ALLERGY TO LOCAL ANESTHETICS OF THE ESTER TYPE].
Precautions
Patients with heart problems, [HIGH PRESSURE], thyroid disorders, [DIABETES], with [PROSTATIC HYPERPLASIA] or under treatment with antidepressants.
Maintain normal bowel function through a proper diet (fruits, vegetables, adequate fluid intake, and regular exercise). It may be helpful to simultaneously take a laxative, bulking agent, or stool softener. Avoid excessive use of laxatives, as this may cause bowel instability.
In patients over 65 years of age and debilitated patients, due to a greater tendency towards systemic toxicity, the dose should be reduced.
It is not recommended for use in children due to limited data on efficacy and safety.
Adverse reactions
- Nervous system disorders (very rarely): Hypersensitivity reactions or systemic manifestations ([NERVOUSNESS], [ANXIETY], [HEADACHE], [INSOMNIA]) especially in predisposed individuals.
- Cardiac disorders (very rarely): [PALPITATIONS]. Rarely: Topical administration for prolonged periods may cause [ISCHAEMIC HEART DISEASE] due to the vasoconstrictor action of ephedrine.
- Others: Rarely: Anaphylactoid reaction (consisting of [CONTACT DERMATITIS]), anal burning sensation and very rarely metoglobinemia characterized by cyanosis.
Prolonged contact of benzocaine with mucous membranes may cause dehydration of the epithelium and hardening of the mucous membranes.
If adverse reactions are observed, treatment should be discontinued and
be notified to Pharmacovigilance systems

- It can cause drowsiness, so it is recommended to be cautious when driving and not to combine it with medications or other sedative substances such as alcohol.
- Any changes in the patient's behavior or mood should be reported to the physician.
- The doctor should be notified if the patient is being treated with antidepressants.
- If after a one-week treatment period the cough persists or is accompanied by severe headache, fever, or rash, it is advisable to consult a doctor.
- It is advisable to drink plenty of water during treatment.
